Transforming care – for those who need us most
A person with complex needs has every right to expect tailored, specialist nursing care. Yet far too often, they’re placed in settings which aren’t able to truly meet their needs.
They may be admitted to, or remain in hospital; be placed in a general nursing care home; or move into a hospice, where capacity is limited. This places pressure on everyone: the individual and their family; professionals who are caring for them; as well as commissioners for care services.
We’ve listened to what health and social care professionals tell us is needed, and responded by creating a purpose-built home that provides a real alternative to home, hospital or hospices.
Woodside provides 24-hour specialist nursing care, for people from middle to old age. This includes people who require:
- complex short to medium term nursing care (for example, those awaiting assessment or who wish to avoid hospitalisation, people with complex or unstable needs or who need respite or convalescence);
- middle to older age people who have severe and enduring medical conditions that result in long-term physical disability;
- end of life or palliative care.
Person-centred care is at the heart of our approach. We take time to identify an individual’s nursing needs so we can offer the best possible care. We also act on their choices, so that as far as possible they:
- continue their daily habits and lifestyle
- maintain or maximise their remaining abilities
- remain part of the community
- feel that their families and friends can be part of our home
- preserve their sense of identity, control - and hope
The result? The right environment and care for these
residents, and a high quality, cost-effective service for
commissioners of care services and self-funding
individuals.
